I just got my XFX 6600GT last night and had the same problem until I read your post. Plugged it in and works fine now.
This is the after-thought addendum page that fell out of the installation manual (crap!) when I finally got around to looking through it.
"Addendum - XFX strives to bring you the best performance as usual. For better display quality, we strongly suggest you connect the 4-pin power connector from the power supply to the VGA board."
That's like being Ford and saying "For better performance of your new car, we strongly suggest putting the key in the ignition and starting the ignition."
