Hlp Me Pls! - Fan Replacement

I have a Geforce 4 Ti4200 and have recently been having overheating problems. I identified the problem - the cards fan is not working at all.

 

I know it is possible to get a replacement fan, but where do I buy this, and more importantly how do I put it in?
